What You Can Expect On Your First Visit

Initial Examination

Your dentist will start by reviewing your medical and dental history and performing a comprehensive dental exam. Your exam will include an in-depth examination of your gums, teeth, jaw and bite, any required x-rays and an oral cancer screening. 

Treatment Plan

Your dentist will create a long-term dental treatment plan for you based on your history and examination. He or she will then take time to answer any questions you may have about your oral health and treatment options.

• How can I book an appointment?

  Please contact our office at Evergreen Dental  during our regular business hours to book an appointment.

• How do I cancel or reschedule an appointment?

  To cancel or reschedule an appointment, please call us. Please notify us of any change in plans as soon possible, so we can offer the space in our schedule to another patient.

• Do you offer appointment reminders?

  In the days leading up to your appointment, you will receive automated phone, text or email appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible.

• Does your clinic have on-site parking?

  Yes, there is free onsite parking available to all our patients. You can also access our location easily via public transit. 

Insurance & Financing

At Evergreen Dental, we'll guide you through our options for payment, direct billing to insurance companies and financing so you can focus on getting the dental care you need.

• Direct Billing to Insurance

  We offer direct billing for most insurance plans. For your convenience, we can submit your insurance claim electronically, resulting in faster reimbursement, without you needing to fill out paper forms and signatures.

  Our administrative team can assist you in understanding your insurance plan better and can also send estimates to your insurance company before starting any major treatments. 

  Please bring your insurance plan information to your appointment.

• Financing Through Dental Card

  Evergreen Dental offers financing options through Dental Card help you manage your  dental payments at an affordable and comfortable pace.

• Payment Methods

  You will be responsible for any amounts or co-pays not covered by your insurance. These must be paid at your appointment time. 

  We accept payment by debit, cash and most major credit cards.
